To help prepare you for your visit to Family Health Partnership Clinic, we have provided some information about your visit.

 

Appointments are scheduled ahead of time, there are no walk-in appointments available at the clinic.  Appointments may be made at 1-815-334-8987, ext. 10.  Patients are usually seen within 3-4 weeks of calling for an appointment.  There is no urgent care available at the clinic.

 

Patients should bring to their first appointment:

   Forms filled out (if possible)

   Photo I.D.
